Aims and goals

The project deals with bridging gaps in care at the interfaces of medical rehabilitation. Of particular importance is successful transition management that prepares and supports transitions to further medical treatment, everyday life and work at an early stage and in line with individual needs. In this way, the success of rehabilitation can be ensured and rehabilitation patients can be enabled to achieve their long-term participation goals. For insured persons of the pension insurance, there are still no uniform standards in this area, as they already exist for statutory health insurance. The project therefore aims to develop a general concept with ready-to-use tools for transition management in the German Pension Insurance’s medical rehabilitation.
