Post-COVID syndrome: Optimization of access to rehabilitation and needs-based medical rehabilitation in Bavarian rehabilitation facilities - POSCOR
Aims and goals
The POSCOR project aims to improve the care of persons with Post-COVID-syndrome in the region of Lower Franconia (Bavaria). It coordinates and promotes cooperation between general practitioners, four specialized rehabilitation centers and the German Pension Insurance (Deutsche Rentenversicherung) as the rehabilitation provider.
Methods
Using telemedical methods such as the website www.poscor.de and the smartphone/tablet app „POSCOR", patients are supported in their waiting time until the start of their rehabilitation through resource-oriented immediate measures and self-help tools, such as breathing exercises, as well as extensive knowledge transfer. In addition, the application process for rehabilitation is simplified so that a suitable rehabilitation can be applied for quickly and easily using a smartphone.
