Pharmacogenetics for personalized pharmacotherapy

The efficacy of drugs varies from person to person. For the best individual selection of a psychotropic drug and its optimal dosage, the Department of Psychiatry, Psychosomatics and Psychotherapy at the University Hospital of Würzburg offers genotyping of CYP2D6 and CYP2C19. These influence the metabolism of various psychotropic drugs.

Why genotyping?

People react differently to drugs. In addition to individual factors, for example age, gender, weight, smoking status, or concomitant medication, genetic characteristics also play a decisive role. The two genes CYP2D6 and CYP2C19 form enzymes that strongly drug metabolism. Variants in these genes give rise to variants in the enzymes with different activity. Accordingly, ingested active ingredients are metabolized rapidly, normally or slowly. Therefore, the dosage or the selection of a drug should be based on the individual genetic predisposition in order to avoid over- or underdosing and to ensure a constant level of the active drug.

Determination of CYP genes

In our pharmacogenetic laboratory, we determine the CYP2D6 and CYP2C19 genes and their variants. From this, it can be deduced whether the intended psychotropic drug is suitable for a patient, how high the dosage should be set and whether side effects have to be expected. The information serves the attending physician as a basis for personalized pharmacotherapy, also taking concomitant medications into account.

Procedure and right of objection

For genotyping, only a blood sample is required as part of a routine blood sampling. The findings are used exclusively for the individualized selection and dosage of drugs. Consent to genotyping can be revoked at any time.
The informed consent form is integrated in the consent form for the respective group of persons.

In-house laboratory for pharmacogenetics (PGx laboratory)

In order to enable personalized pharmacotherapy, the Department of Psychiatry, Psychosomatics and Psychotherapy at the University Hospital of Würzburg has set up its own laboratory for pharmacogenetics. The strict requirements of the Gene Diagnostics Act (GenDG) are adhered to. Among other things, this prescribes appropriate counseling and its documentation.
