Nationwide model project
This interdisciplinary outpatient heart failure clinic is nationwide unique. Due to the possibility of working in such an interdisciplinary team together with the university clinic Wuerzburg our patients benefit from modern diagnostic strategies and a tailored therapy. This fulfils our requirement to also advice patients with more severe or complex forms of heart failure.
Who is eligible for treatment?
The interdisciplinary outpatient heart failure clinic is open for all patients in the region. Especially in cases with severe disease it can be beneficial to make use of the expertise in our outpatient heart failure clinic in addition to the regular visits within the cardiology practice.
No emergency department!
Please note, we are no emergency department. In case of severe health conditions, e.g. breathlessness or chest pain, please call the medical service (Phone: 116 117), the rescue directing centre (phone 112) or please visit the next emergency department.
Information for patient admittance
An appointment in our outpatient heart failure clinic within the CHFC can be made by phone: + 49 931 201-46301. Appointments can be made Monday to Thursday from 8 a.m. until 4:30 p.m. and on Fridays from 8 a.m. until 3 p.m.
First presentation in our outpatient heart failure clinic
If planning your initial presentation in our outpatient heart failure clinic it can be useful, especially in the case of a long medical history, to send us all your medical records in advance to carefully plan your visit and to avoid unnecessary examinations. This can be done by post or by fax. Please send your medical records to:

Which documents are required?
Please bring the following to your medical appointment:
- Health insurance card
- Referall slip issued by your primary care physician
- Up to date medication regime
- Current laboratory results
- Documentation of your body weight
- Blood pressure results (if available)
- Physician letters /results
- Medical findings (coronary angiography reports, exercise ECG)
- X-ray pictures, CT or MRI scans, if possible as CD or DVD
- Blood sugar results (if available)
Where to find the heart failure outpatient clinic?
The CHFC outpatient clinic is located on the university clinic campus in building A 15, level -1.
Specialized consultations and consultation hours
In collaboration with our colleagues from the cardiology department within the university clinic we are able to offer specialized consultations. Appointments can be made, if not noted differently, by calling our outpatient clinic secretary (+49 931 201-46301 or via Email: dzhi@ ukw.de

Further specialized consultations
Few specialized consultations as well as the private outpatient clinic are located in the neighboring center for internal medicine (ZIM). The outpatient clinic is located in building A3, level -2. Please register at counter 26.
